Eligibility
In order to be eligible for Athletics, please complete the following items:

• Register for your sport through Family ID.
• Pay your fees: MHS students-athletes are required to pay a $45 Participation Fee and $50 ASB Fee.
  • Fees can be paid online here or you can pay at our Business Office.
• Have an Up-to-Date Physical on File with the Athletic Office - sports physicals are good for two years.
  • Physical Forms are available on our website, on Family ID and on the WIAA website.
  • WIAA Pre-Participation Physical Form

Please note that student-athletes are not eligible to participate until the above steps have been completed and approved by the school nurse.
